Osteosynthesis of proximal femur fractures by the proximal nail of the anti-rotatory femur (PFNATM)

Introduction: Fractures of the proximal end of the femur are serious. Treatment often involves osteosynthesis, sometimes using a PFNA nail. The aim was to analyze the operative data and evaluate the results of this osteosynthesis since its implementation at the Centre Hospitalier Universitaire Régional de Ouahigouya. Methodology: This was a 36-month retrospective study covering the period from June 1, 2021 to June 30, 2024, and including all patients osteosynthesized with PFNA nails for proximal femur fractures in the surgery department. Results: We enrolled 25 patients, 14 women and 11 men, for a sex ratio of 0.78, with a mean age of 65.4 years. Twenty-three patients had a normal Parker score and two patients had a Parker score of 8. Domestic accidents dominated the circumstances of trauma in 13 cases. The 25 fractures were divided into 19 stable and 6 unstable fractures. For PFNA nail osteosynthesis, 21 standard PFNA nails and four long PFNA nails were used. At a mean follow-up of 16.84 months, all patients had consolidated. There were two cases of hardware disassembly: two cases of recoil of the helical blade requiring revision surgery were noted. No in-patient deaths were recorded. Functional results assessed according to the PMA score were good or excellent in 22 cases. Eighteen patients regained their pre-fracture autonomy. Conclusion: Osteosynthesis with the PFNA nail produced good and excellent results in the majority of patients, confirming that the PFNA nail is a good treatment option for fractures of the proximal end of the femur, including the trochanteric and subtrochanteric region, at the Ouahigouya hospital.
